2021-2022 EOFY - OnePath Customer FAQs

OnePath will provide you with:

  • An Insurance Premium Summary if your policy has an Income Protection cover held outside super or the policy is owned by a Self-managed Super Fund

or

  • A Notice of intent to claim a tax deduction for personal super contributions form if:
    • Your insurance is paid through your superannuation; and
    • You have made personal contributions during the 2021-2022 financial year; and
    • The policy is still in force at 30 June 2022.

For those customers who are eligible to receive a Notice of intent to claim a tax deduction for personal super contributions form, we will begin mailing these forms from early August.

The Insurance Premium Summary will assist you to complete your 2021-2022 tax return. However, the information contained in this Summary does not constitute tax advice and we recommend you seek independent tax advice specific to your personal circumstances, from a registered accountant/tax agent.

You can login or register here to the customer portal, My OnePath Life, to access a copy of your 2021-2022 Insurance Premium Summary or Notice of intent to claim a tax deduction for personal super contributions form, depending on which one is relevant to you.

These documents will be available in the portal usually within a few days after we have sent the document to you.

You can login or register here to the customer portal, My OnePath Life, to obtain a copy of your 2021-2022 financial year document(s), whether you are looking for an Insurance Premium Summary or the Notice of intent to claim a tax deduction for personal super contributions form.

After logging in:

  • Navigate to the Insurance section, select your policy and look for Documents;
  • In the Documents drop down menu, select Correspondence to display the past 12 months of correspondence in relation to the selected policy number;
  • Select either Contributions Tax Statement for the Notice of intent to claim a tax deduction for personal super contributions form or Insurance Premium Summary.

Remember a copy of these documents will not be available in My OnePath Life until a few days after we have sent it to you.

While you are in My OnePath Life you can update your mailing address for future reference, or any other contact details that might not be correct.

Yes, your financial adviser is able to access a copy of any of the documents we send you, by logging in or registering for the adviser portal that OnePath provides for them.

These documents will be available to your financial adviser in our adviser portal usually a few days after we have sent the document to you.
