I continuously elect for OnePath Custodians or any successor holding this policy insuring me to take out and maintain insurance, subject to meeting the policy terms including premium requirements under the policy, even if they receive no amount in respect of the policy for a continuous period of 16 months or more.

I acknowledge that I’m making this election under superannuation law and that I can cease the policy on request.
