How to make a claim

To make a claim on OnePath Group Insurance, please contact your employer or super fund.

If your insurance is through the following super funds, you can contact them via the below links:


Do you hold insurance under ANZ Smart Choice?

If so and you wish to make a claim, please call 133 665, option 2.


How does the claims process work?


You complete a claim form

Your employer or super fund will send you a claim form that you will need to complete. This form will outline what information we need to assess your claim. The sooner you send us that information, the sooner we can assess your claim.



We contact you

Once we’ve received your claim forms, we will contact you directly to discuss your claim, provide advice about the claims process and answer any questions you may have.



We assess your claim

Our claims team will assess your claim as quickly as possible. We will let you know if we need any further information from you or your doctor(s).



How we pay a claim

We will inform the policy owner (your employer or super fund) about the outcome of your claim, and they will get in touch with you.

For eligible claims, the way we pay the claim depends on the type of cover. For example:

  • Group Life benefits such as Death, Terminal Illness or TPD benefits are generally paid to the policy owner who will pass the benefit on to you or your nominated beneficiaries.
  • Group Salary Continuance benefits are generally paid direct to you. In some cases, however, we will pay benefits to the policy owner who will then pay those benefits to you.
